Autism Disorder Labels May Leak Out

How many children are actually autistic and what about those who are diagnosed with a milder form of autism?

Both questions go to the heart of the controversial debate about how autism should be classified.

Labels such as autistic spectrum disorder (ASD) and high-functioning autism have often been criticized for being vague. They indicate a diagnosis that can apply to children who don’t meet strict criteria for an ASD and those who do.

The diagnostic categorization could have unintended consequences for families. It can lead to loss of government benefits, few job opportunities, less support for the child’s development and other problems in life, according to researchers at the University of Missouri and other institutions.

“A lot of people are not sure what to think about it,” said Peter Welinder, a University of Missouri associate professor of child and adolescent psychiatry and co-director of the Translational Autism Center.

He and his colleagues published their first study on the topic in April 2013 in the journal Pediatrics.

The study, which involved 13,000 children ages 3 through 17, found that parents of children with severe intellectual disability who were labeled with an ASD diagnosis had a higher likelihood of divorce, moving out of the family home, increasing their child’s use of drugs or alcohol and failure to thrive.

More than a third of the children — who could have mild to moderate intellectual disability but not meet diagnostic criteria for an ASD — scored in the mild to moderate range on the autism spectrum scale, the researchers found.

The researchers said they didn’t observe negative consequences of either of the labels for children who scored in the mild range on the Autism Spectrum Screening Questionnaire, a test used to identify children with possible ASD.

“We actually found that the kids who are higher functioning have better outcomes,” Welinder said.

Welinder said his study has received a lot of attention for raising the alarm about potentially negative consequences for children and families.

“People do have a concern for the impact that these labels may have on families,” Welinder said. “But this is really about just that parents have the right to make the best decision for their children.
