The gameplay is similar to the console games and follows the same rules as Mortal Kombat 3 (with the single-player modes being essentially the same as in Mortal Kombat 3, but with more cinematic sequences and the addition of puzzles, although puzzles are not necessarily required to complete the games). Mortal Kombat 9 and the Kung Lao: Dead Men Imitating Life story are the only fatalities on the PS2 version. While the story begins a few years after the previous Mortal Kombat games, some events have returned to occur. For instance, the survivors of the first tournament escape Outworld and gather on the living forest where the events of Shaolin Monks take place, and, the player controls Liu Kang and Shang Tsung throughout the story. It is unknown whether Shang Tsung is living on Earth-realm and Outworld at this time.

The game features fatalities and several other fatalities. However, it was later revealed that only two of the fatalities would be included in the game: Noob Saibot’s fatality against Alisa Bosonic, and Baraka’s fatality against Scorpion. Some fatalities from previous games were also removed: Jade’s normal Fatality against her wasn’t included as she was still alive, yet, she was killed by the player’s options in the third Mortal Kombat film. Rain’s Kiefer finisher in Mortal Kombat 3 was made available. In addition, the Mortal Kombat 3 Raiden’s Fatality against Sub-Zero was unlocked, although Sub-Zero gets his right hand back and is still able to perform a Fatality.

The gameplay revolves around Liu Kang and Kung Lao that focus on fighting with weapons and efficient martial arts, respectively. Co-op allows you play as a fighter in melee range, while Special Moves or Takedowns enable the two characters to engage enemies at medium range. The game has the player taking control of Liu Kang or Kung Lao, who are seemingly the only two fighters in the game.
